Integrating Health and Social Care

In many countries, health and social care are managed in different sectors. Data are often not shared between the two sectors and problems occur, when social factors impact a patient’s health – and the other way around. Citizens and patients, and the health professionals and social workers who meet them, are victims in this fragmented system. Citizens and patients must be at the centre to ensure continuity of care in its entirety, both from a social and a health perspective.
In this conference, we focus on the experiences and challenges – organisational, legal and digital – we meet when trying to promote data sharing between the two sectors, seen both from a Danish and an international perspective. We take a look at strategic and practical approaches and we hear from policy makers as well as from the everyday life in a municipality, for which this is a real life issue.
